Death happens, but named characters should not die cheaply. Barring exceptionally special circumstances, you control if your character would perish or not in a particular perilous instance. If you're thinking of killing your character, talk with staff about it. They may suggest that you not do so, but they may also step up to help you have a properly heroic or villainous or neutralicious death.
This is somewhat less true for NPCs, although even there, remember that a little mass death goes a long way.
In the vein of this, permanent injuries should be handled in a similar manner. Break a leg? Fine. Have a leg fly off, get caught by a fan, and then get used to drink beer out of? Not so much. Think very hard about whether you really want this character to be a cripple.
